  • بیماری کرون یک بیماری مزمن است که قسمت‌های گوناگون دستگاه گوارش از دهان گرفته تا مقعد را می‌تواند تحت تأثیر قرار دهد. این بیماری بیشتر قسمت انتهایی روده باریک (ایلئوم) و قسمت ابتدایی روده بزرگ را تحت تأثیر قرار می‌دهد. این بیماری می‌تواند تمام لایه‌های روده را درگیر سازد و حتی موجبفیبروز شود. شایع‌ترین ناحیه درگیر ایلئوسکال است. بین ضایعات معمولاً نواحی سالم وجود دارد. این بیماری تظاهرات خارج روده‌ای نیز دارد مانند آرتریت و ضایعات پوستی.

    علائم و نشانه‌های آن اغلب شامل اسهال (که در صورت التهاب شدید روده ممکن است به اسهال خونین تبدیل گردد)، تب، کاهش وزن و درد شکم است. برخی از عوارض این بیماری ممکن است در خارج از دستگاه گوارش رخ دهد، شامل کم خونی، خارش پوست، ورم مفاصل، التهاب چشم و احساس خستگی. از دیگر علائم این بیماری، راش و اریتم گرهی است. انسداد روده نیز معمولاً رخ می‌دهد و کسانی که به این بیماری مبتلا می‌شوند بیشتر در معرض خطر ابتلا به سرطان روده قرار می‌گیرند.

    A case published in the New England Journal of Medicine shows exactly why sunscreen is a must – with a case of a 69 year-old truck driver, who drove for 28 years with the left side of his face exposed to the sun, while the right side faced inside and remained protected.
    .
    When compared to the right, the exposed side shows the effects of chronic sun exposure: "hyperkeratosis with areas of accentuated ridging, multiple open comedones, and nodular elastosis." In non-medical terms: thickened skin, deep wrinkles, enlarged pores, and irregular texture. This photodamaged state is referred to as dermatoheliosis.
    Although UVB rays are more carcinogenic, both UVA and UVB rays, with enough exposure, have been shown to mutate DNA in skin cells and begin the process of skin cancer. So, sunscreen doesn't just protect us from sunburns. Sunscreen protects us from the effects of chronic UVA/UVB exposure, clearly seen within this single individual in the image above, drastically reducing the risk of skin cancer later in life.

    Thus began an intense journey for the USMLE exams. It took a lot of dedication and hard work to sit through books studying content taught to us in medical school all over again. All an effort to prove ones worth, and show we were equipped with adequate knowledge to pursue the arduous journey of residency

    Despite all the planning and preparation on how I had envisioned my career timeline, life had other plans for us. Two months after moving to the USA, we found out we were pregnant. This is normally an exciting moment for people, but I was close to the verge of tears. I was in a strange new country, faced with massive cultural adjustment, miles away from home and no health insurance. It was the worst time ever to be bringing a tiny human into this world. Not to mention the looming exam deadline slowly creeping up on us

    My uncle and aunt were so comforting and reassuring, but it was still a huge change, and with so much uncertainty prevailing, the thought of having a baby was terrifying. “Here we were still struggling to get our feet wet and figure out stuff, and now we were having a child”- such a huge miracle, but one that also came with its own set of responsibilities that only we could be answerable for. “How were we going to pull through?”

    Days turned into weeks, and it wasn’t an easy transition. How I yearned to celebrate the joy of my first pregnancy amidst family and close friends, and use every moment to bond with my baby. Instead I was faced with the stresses of studying, adjusting to a new life, new culture, and winter beginning to creep in on us

    Each waking day I wished I could go back in time, hear the laughter and chatter in the air during family events, and appreciate the beauty and warmth of everything I knew as home- a constant reassurance that everything would fall into place

  • Today s case:  a 60 year old male presented with symptoms of increased urinary frequency and nocturia. .

    On  Per rectal examination👈🏻 the surgeon found the prostate to be enlarged and PSA tests were which came out be increased . A biopsy was done and sent to pathology department for evaluation. > >

    Interpretation: 👀Slide 1: The image shows numerous glands separated by intervening stroma (back to back glands with lack of intervening stroma is a feature of malignancy)

    . 👀Slide 2: shows an irregularly shaped gland with papillary infolding.Now this feature is normal and indicates a benign gland, as in a malignant case the glands in prostate become round, small and tubular and lacks these papillary infoldings in prostate carcinoma. .
    .
    👀Slide 3: Shows presence of both basal and luminal epithelial cells (lack of basal cell is a feature of carcinoma). The epithelial cells also lack atypia, mitosis and hyperchromasia (again a feature of malignancy). .

    Based on the above findings diagnosis of Benign prostatic hyperplasia 💡(BPH) was given. .

    . . 📘Slide 5 shows how a gross of benign prostatic hyperplasia looks like where the gland is increased in size (normal weight 20gms) and shows nodules which are smooth and grey white in color. .
